Submit a report of the development project to the SDA Awards Administrator within one month following the conclusion of the project (if the project extends beyond one year, six-month progress reports are required). Each report should identify all expenses incurred within the six-month period and provide copies of invoices, receipts, or cancelled checks documenting those expenses.

Submit a completed W-9/W-8 form prior to receipt of grant funds. If this requirement presents an issue, please contact us at awardsadministrator@surfacedesign.org

Prepare and submit a brief reflection describing how the grant supported your role as an educator within 60 days of the project’s end date.

If your proposed educator grant is funded but does not take place, the full grant amount must be returned to SDA.
